Total Hip Replacement (THR) or Hip Arthroplasty is an operative procedure in which the diseased and destroyed hip joint is resected and replaced with a new bearing surface. In case of end-stage oseteoarthritis, THR is a definite treatment plan for patients.

Indication

  • Osteoarthritis
  • Avascular necrosis of the head of femur
  • Ankylosed hip
  • Failed hemi replacement arthroplasty
  • TB of hip
  • Rheumatoid arthritis

Procedure

In THR, the acetabular as well as the femoral head components are replaced. Two types of implants are used:-

  1. Cemented Implants:- In the cemented hip, the acetabular as well as the femoral components are fixed into place with the help of bone cement. Mainly used in early patients where bone stock is poor.
  2. Non-cemented hip:- In this, the acetabular component is fixed into place with the help of screws and femoral component is fitted snugly into the medullary canal of femur. Mainly used in young patients where the bone stock is good.

Two types of surgical methods are mainly used:-

  • Posterior approach is the most common approach used by the surgeon as no muscle are cut during surgery and excellent exposure of both femur and acetabulum is seen.
  • Anterior approach is made through the front of the hip using the interval between tensor fascia lata and sartorious. It has good long-term results.

Total Hip Replacement Recovery

Patients will be given pain medicine as well as antibiotics to avoid infection after surgery. Physical therapy is a crucial component of the rehabilitation process. You will collaborate with a physical therapist to create an exercise and rehabilitation plan until you are able to return to an active lifestyle and walk independently.
The length of your hospital stay will be determined by a variety of criteria, including pain control and medical stability. Normally, it takes 4 to 5 days.

Complications

  • Aseptic loosening (most common)
  • Deep venous thrombosis
  • Vascular injury
  • Dislocation
  • Infection
  • Heterotopic ossification
